WELCOME to the Humane Society of Morgan County Adoption Application!
We are glad you have come to adopt a new pet from our shelter.
The following information is requested so that we can:

1. Assist you in the selection of a new pet;
2. Determine if the adoption is in the animal's best interest; and
3. Assist you in finding an animal most compatible with your lifestyle.

In order to be considered as an adopter, you must:

1. Be 18 years of age or older;
2. Have identification showing your present address;
3. Have the knowledge and consent of your landlord;
4. Be able and willing to spend the time and money necessary to provide training, medical treatment, and proper care for a pet;
5. Keep the animal as a family pet;
6. Maintain the animal in compliance with the County"s leash law;
7. Not adopt the animal as a gift;
8. Not keep the animal on a chain;
9. Spay or neuter the animal;
10. Have a current County license;
11. Keep the animal current on rabies vaccinations;
12. Keep the animal in compliance with State Laws;
13. Not give away or sell the animal but must return it to the shelter if you can no longer keep or maintain the animal properly.

The Humane Society of Morgan County reserves the right to investigate new homes and remove the animal if it is not receiving adequate care, has been mistreated, or the spay/neuter requirements have not been met.

Our adoption fee includes: spay/neuter (or credit if animal is too young), first series of vaccinations, rabies vaccine, felv test (cats), and de-worming for internal parasites. All adopters are placed on our mailing list.
